Pudendal neuralgia and related chronic pelvic pain are often described as burning, aching or electric pain in the perineum, genitals or rectum, sometimes with numbness or a sensation of sitting on a foreign object. Many patients report that the pain is affected by sitting and eases when standing or lying down, although this pattern varies from person to person. Urinary urgency, bowel discomfort and sexual dysfunction may accompany the pain.
How is pudendal neuralgia diagnosed? Assessment at the Dubai centre begins with a structured history and clinical examination, usually interpreted alongside the Nantes criteria. Depending on the findings, pelvic MRI or MR neurography, pelvic floor assessment and a diagnostic pudendal nerve block may be used. Because no single investigation confirms the diagnosis in isolation, results are weighed together, and other causes of perineal pain are considered before surgery is discussed.
Treatment is stepwise. Pelvic floor physiotherapy, neuropathic pain medication and image-guided nerve blocks are considered first. Where symptoms persist and the clinical picture suggests nerve entrapment, laparoscopic pudendal nerve decompression may be offered. How long should I plan to stay in Dubai for surgery?
